  • Hourly/Daily Attendance Report for Regular Employee
    Payroll must receive this form with an authorized signature, date signed and employee signature with date signed before it can be processed.

  • Hourly Attendance Report for Non-Permanent Reassignment
    Payroll must receive this form with an authorized signature, date signed and employee signature with date signed before it can be processed.
    

• Annual Request for Vacation Pay-off Form

  Payroll must receive the Annual Request for Vacation Pay-off Form with each line completed and with an original signature by May 15th. Only permanent Aides, OSS or OTBS employees on pro-rated calendars are eligible to complete this form.
